WASHINGTON (Reuters) - A U.S. Secret Service probe found communication gaps and a lack of diligence ahead of the July 13 shooting at a rally of Republican presidential candidate Donald Trump in Butler, Pennsylvania, in which the former president was shot and a rally attendee was killed.
